Theresa, WI — Police have identified the victims of a fatal chemical incident at a Dodge County manufacturing plant that left one man dead on Monday, November 28, 2011. Toxic fumes killed 55-year-old Roman Torres of Horicon and injured Thomas Persha, 65, also of Horicon at Vivid Image Inc., reported the Fond du Lac Reporter.

Torres was pronounced dead at the scene after Persha’s wife found the two men unconscious at the plant. They were reportedly the only two people in the building at the time of the incident.

Persha was rushed to a Hartford hospital for treatment of his serious injuries.

Police officials reported that the workplace accident occurred as the victims were mixing two or three compounds when the fumes apparently overcame them, as previously reported in “Chemical Fumes at Wis. Plant Kills Worker, Injures Another.”

First responders also noted a strong chemical smell upon entering the building.

Investigations into the workplace accident are underway. Officials with the U.S. Occupational Safety and Health Administration (OSHA) are expected to arrive at the scene to conduct their own investigation.
