San Pablo Man Killed in Industrial Accident at Berkeley Tool Plant

Berkeley, CA — An industrial accident killed a San Pablo man Thursday night, March 15, 2012, at a Berkeley metal forging and tool-manufacturing plant. The fatal workplace accident occurred at Berkeley Forge and Tool, located at 1330 Second St., at 10:23 p.m., reported the San Jose Mercury News.

The technician, Ariel Munoz-Garcia, 30, was fatally injured after a section of a metal grinding wheel hit him in the back of head.

The California Division of Occupational Safety and Health (OSHA) says the wheel shattered and sent metal pieces flying, the San Francisco Chronicle reported.

OSHA is looking into whether the grinder had proper guards and was properly maintained and what training, if any, Munoz-Garcia had received to use it.

“Our investigator will look at the site and the equipment the individual was working with and do an inspection of it,” OSHA spokesman Dean Fryer said to San Jose Mercury News. “We’ll interview other employees in regards to training and maintenance issues, we’ll talk to the employer about any safety measures that they should have in place. We also want to look at the equipment itself.”

Investigations into the fatal industrial accident are underway.
